upcoming weekend phlebotomy certification class on campus


I am letting you know about an upcoming weekend phlebotomy 
certification class on campus. Please review the details and required documentation if you are interested.

Details

About. Training is 2 days, Sat July 21 & Sun July 22 from 9-5pm in Campus Health Rm B307. Please arrive by 8:15am on Saturday to check-in. On Sunday, class usually finishes a little earlier around 2:30pm, with an optional discussion afterward about getting into med school and job/volunteer opportunities as a phlebotomist. I recommend checking out the reviews, he is a very experienced instructor.

Cost. The total cost is $335 (including the $80 deposit), which covers classtime, text, and a mock medical kit. Acceptable payment on class day includes cash, cashier's checks* (make payable to MET Services), and money orders.  *Please note personal checks and venmo are currently not being accepted on class day. Snacks are provided, although you will need to bring your own lunch.

Sign-up. 
1. Sign up here to get on the class list. 
Registration is based on a first come, first serve basis, not from the numbering system on the sign-up sheet. The capacity is 30 students. I will be providing you updates on the sign-up sheet as time progresses as far as the number of seats remaining goes.
2. Fill out the attached registration form and turn in your $80 deposit
Payments are currently being accepted digitally via venmo @Kassandra-Diaz-10, and in-person via cash, personal/ cashier's check (make payable to MET Services), or money order. 
3. Wait for an email confirmation from me. 
Both the form and deposit need to be turned in before I am able to confirm your seat. 
4. Read the class orientation instructions that will be sent to you in the week before class. 

Notes about certification.
Certification is valid anywhere in the United States for two years from month of class with the exception of California (there is a different state law), although students have also drawn overseas.
